Medical Forms
All medications, including aspirin, ibuprofin, cough-and-cold medication, decongestants or other over-the-counter or prescription medications require prior authorization from both a parent and health care provider before that medication can be taken or given during the school day, on field trips, or during athletic events or during any other school related activity. A new authorization must be completed at the beginning of each school year and each time there is a change in dosage or time of administration.
Student Insurance Forms
Attached is an optional form of medical and dental insurance for students offered through K&K Insurance Group Inc. If you do not have medical insurance for your student, especially if your student participates in sports, it is highly recommended that this insurance be considered.
